Contact Form
Input your client contact information and inform of us your availability so that we can schedule your initial call.

Initial Phone Call
During the initial phone call and intake, we will collect demographic and insurance information, and connect you to a clinical staff member who will be able to answer many of the initial questions that you might have to determine if Ambrosia is ​a good fit.

Assessment
​
Our 60 minute assessment session at our office with a trained clinician allows us to become familiar with your situation in order to know who you are and what you need, and to be able to make a recommendation for your personal treatment plan.

Insurance Verification
​
Your insurance company will be contacted to verify benefits including co-pay, deductible and or any co-insurance. With this information, we will the communicate your financial responsibilities in a written outline and over the phone.

Medical Clearance
​​
We will provide you with a medical clearance form which requests initial labs to be taken by your primary care physician, in order to verify that you are well enough to be in an outpatient program setting.
06
Admission & Orientation
​​​
We will set your admission date and upon arrival, we will give you a brief tour, introduce you to our clinical staff, and introduce you to the group program. ​​
